Introduction

Our practice is part of the Southend Victoria Primary Care Network (PCN).

Primary Care Networks bring together groups of GP practices to work collaboratively with other healthcare professionals to provide more joined-up services for patients.

 

What is a PCN?

A Primary Care Network (PCN) is a group of local GP practices that work together with community services, pharmacies, social care, and voluntary organisations.

The aim is to:

· Improve access to healthcare services

· Provide more coordinated care

· Offer a wider range of services closer to home

· Focus on prevention and supporting long-term health

 

How the PCN Supports You

Through our PCN, you may benefit from:

· Additional clinics and services delivered across local practices

Our PCN includes a range of healthcare professionals who work alongside GP practice teams, such as:

· Clinical Pharmacists

· First Contact Physiotherapists

· Social Prescribers

· Care Coordinators

· Health and Wellbeing Coaches

· Physician Associates

These professionals may be involved in your care depending on your needs.
